Developed in collaboration with the American Stroke Association, The Joint Commission's Certificate of Distinction for Primary Stroke Centers recognizes centers that make exceptional efforts to foster better outcomes for stroke care. Achieving certification signifies that the services these hospitals provide have the critical elements to achieve long-term success in improving outcomes. It is the best signal to the community that the quality care they provide is effectively managed to meet the unique and specialized needs of stroke patients. Demonstrating compliance with these national standards and performance measurement expectations may help obtain contracts from employers and purchasers concerned with controlling costs and improving productivity. Advanced Certification in Heart Failure
The Joint Commission, in collaboration with the American Heart Association, has developed the Advanced Certification in Heart Failure to distinguish those facilities demonstrating exemplary heart failure care. A commitment to continuous improvement is essential. The Advanced Certification in Heart Failure requires, at a minimum, that a program collect data on the four Joint Commission core measures for heart failure and use this information for ongoing performance improvement efforts.
GWTG-HF is a prerequisite to obtaining this certification. Organizations are eligible to apply for this advanced certification with GWTG-HF Bronze Level recognition or higher. Learn more
